Claim submissions for Culinary Protection can be made by filling out a virtual form on the claim submission media that will be provided by igloo through the DANA application portal. The required claim documents are as follows:
1. Claim Form
2. Insurance Certificate
3. Identity Card (KTP)
4. Medication Receipt (alternative treatments are not applicable)
