Submitting a claim for Mobile Screen Protection can be done by filling out a virtual form on the claim submission media provided by the Insurance Broker Partner through the DANA application portal. The required claim documents are as follows:

1. Name of Insured

2. Mobile Brand

3. Mobile Type

4. IMEI Number

5. Damage Chronology

6. Photo of the damaged phone screen

7. Photo of ID card

8. Receipt of repair costs
