Claim Procedure - (Pet Insurance)

All claim notification must be made within 72 hours since date of accident on www.sinarmas.co.id

a) In case of Accidental Death
1. Chronological report of events
2. Genuine Stambum (Dog) or ICA Certificate (Cat)
3. Dog/cat purchase receipt ((If the purchase receipt is not available, then the replacement is according to the price of the insurance partner's pet shop)
4. Photocopy of policy, certificate, attachments/endorsements
5. Claim form
6. Death certificate from IKK (Indonesia Kennel Klub)
7. Veterinary certificate containing the death due to an accident
8. Other certificates required at the discretion of the insurer

b) In case of loss due to theft by force
1. Chronological report of events
2. Genuine Stambum for dogs or ICA Certificate for cats
3. Dog/cat purchase receipt
4. Photocopy of policy, certificate, attachment/endorsement
5. Claim form
6. A statement from the authorities/police about an accident or loss
7. Other certificates required at the discretion of the insurer

c) In terms of third party legal liability
1. Chronological report of events
2. Genuine Stambum for dogs or ICA Certificate for cats
3. Dog/cat purchase receipt
4. Photocopy of policy, certificate, attachments/endorsements
5. Claim form
6. Receipt of payment of compensation
7. Letter of defense and/or terms of negotiation
8. Letter of claim and letter of acknowledgment from the insured
9. Other certificates required according to consideration of the guarantor

d) In case of additional guarantees (Hospitalization Compensation and Cremation Compensation)
1. Original receipt for hospital treatment
2. Diagnosis of disease
3. Details of detailed costs
4. Photocopy of medical resume
